NSID certification requirements

IMPORTANT – PLEASE READ BEFORE CONTINUING

Do not proceed with the certification process unless your athlete has already completed registration for the Supreme Team Youth Football or Cheer season.

Certification is required for participation and cannot be completed without prior registration.

ATHLETE CERTIFICATION REQUIREMENT

All athletes must be certified in order to participate in practices and games. Certification status will be verified weekly to confirm eligibility for competition.

Supreme Team uses National Sports ID for athlete certification in compliance with league requirements.

Athletes must be fully certified to practice or compete.

Before beginning, please review the National Sports ID verification video to understand the required steps:


How to Get My Child Verified – 3 Step Process


Once you’ve reviewed the video, create a parent/guardian account and add your athlete by visiting: https://www.nationalsportsid.com/

TEAM LINK ACCESS

Your team-specific National Sports ID link will be provided in your registration confirmation or receipt.

If you have not received your team link, please email info@supremeteamyouth.com before continuing.

REQUIRED DOCUMENTS (READ CAREFULLY)

You must have all required documents ready to upload before starting the certification process.


Do not begin certification until all documents are available.

DOCUMENTS NEEDED FOR CERTIFICATION

 National Sports ID will require the following items to verify your athlete:

  • A clear headshot photo of the athlete
  • Birth Certificate, Passport, or State-Issued ID
  • School document showing the athlete’s current grade
    (Report card, progress report, or official school record)
  • Medical clearance or physical exam documentation from a licensed physician
  • Medical documentation must confirm the athlete is cleared to participate in athletics
  • Physical exam must have occurred within the last two years and be dated 2025
  • Clearance must be signed by a doctor and show no participation restrictions

IMPORTANT REMINDERS

  • Incomplete uploads will delay certification
  • Athletes cannot practice or compete without approval
  • Certification must remain current throughout the season

Please ensure you’ve joined the Supreme Team in NSID for your registered level. This was a common issue that other parents experienced during equipment day and should be an easy fix if you are already registered. Should you still receive an error despite your verification, please reach out to your team's head coach or team mom.


Please re-watch the NSID help video for instructions on how to verify your athlete’s materials. If you still need support, please contact NSID directly through their Live Chat channel in the lower right corner (green box) of their website.


No, athletes will not be able to practice or compete without NSID approval.
